  OVERVIEW  
 

You must register the birth of your child, born overseas to at least one Filipino parent, with the Philippine Embassy. This ensures that their Filipino citizenship is properly recorded with the Philippine Statistics Authority. 

Report of Birth applications must be submitted by personal appearance and by mail.

 
     
 
 
     
  GENERAL INFORMATION  
 

The birth of a child born to at least one Filipino parent should be reported to the Philippine Embassy in Islamabad, Pakistan if it occurred in these areas:

    • In Pakistan: Islamabad, Lahore,  and Karachi.
    • Within Jurisdiction: Afghanistan, Kyrgyzstan, and Tajikistan.

There is no monetary penalty for delayed civil registration of birth. However, it is important to report these events promptly to ensure proper documentation and recording in the Philippines.

Report of Birth applications are processed in the order they are received. If all documents are complete and accurate, your Report of Birth will be processed and dispatched within 7 working days.

The Philippine Embassy assumes no responsibility for any delay or loss during mail delivery, or while the documents are in the custody of the courier service.

 
     
 
 
     
  HOW TO APPLY  
 

1. Prepare Your Documents. Complete your required documents before submitting your application. This includes your cover letter, supporting documents, a self-addressed stamped envelope, and the required money order.

2. Send Your Application. Mail your completed application package to the following address:

3. Await Delivery. Once received, your application will be promptly processed. If documents are complete and in order, the Report of Birth will be processed and mailed out within 7 working days after receipt of the application.

 
 
 
     
  WHAT INCLUDE ON THE MAIL  
 

Please make sure all your documents are complete before mailing your application to avoid delays in processing. The Embassy reserves the right to return incomplete applications.

1. Cover Letter

    • Indicate the service requested
    • Write your name and contact information (mobile number and email address)
    • List the enclosed documents

2. Notarized Report of Birth Form

    • Four (4) original Report of Birth Forms, each notarized by a notary public
    • Must be printed/typewritten. Handwritten documents will not be accepted.
    • Applicants are encouraged to email their draft ROB form to islamabad.pe@dfa.gov.ph before notarization

3. Birth Certificate

    • One (1) original and four (4) photocopies of the child's foreign birth certificate

4. Parent's Passport 

    • Four (4) photocopies of both parent's passport data page

5. Proof of Parent's Filipino Citizenship

    • Four (4) photocopies of Filipino parent's proof of current Filipino citizenship, such as permanent resident card, visa, work permit, or dual citizenship identification certificate
    • (IF PARENT IS A FORMER FILIPINO) Four (4) photocopies of the former Filipino citizen's foreign naturalization certificate

6. Self-Addressed Stamped Envelope

    • (FOR PAKISTAN RESIDENTS) One (1) pre-paid self-addressed courier label and tracking numbers
    • (FOR AFGHANISTAN, KYRGYZSTAN, & TAJIKISTAN RESIDENTS) One (1) pre-paid self-addressed courier label

7. Money Order 

    • USD 25.00 processing fee for Report of Birth
    • USD 25.00 processing fee for Certificate of Registration of Legal Instrument for those executing Affidavit of Admission of Paternity and Affidavit to Use the Surname of the Father
    • Additional USD 10.00 for expedite release
    • Please issue separate money orders for the processing fee and expedite fee
    • Payable to cash only

 

If Parents were Married at the Time of Birth

8. Philippine Marriage Certificate

    • One (1) original and four (4) photocopies of the marriage certificate issued by the Philippine Statistics Authority (PSA)

If Parents Married after the Child’s Birth

9. Joint Affidavit of Legitimation

    • One (1) original and three (3) photocopies of the notarized affidavit
    • Template can be downloaded from here.

10. Certificate of No Marriage (CENOMAR)

    • One (1) original and four (4) photocopies of Filipino parent/s' Certificate of No Marriage issued by the Philippine Statistics Authority
    • If you do not have a CENOMAR, please order online through psahelpline.ph or psaserbilis.com.ph

If Parents Are Not Married and the Father will Acknowledge Paternity

11. Affidavit of Admission of Paternity (AAP)

    • Four (4) original affidavits, each notarized by a notary public and apostilled by the competent authority
    • Executed by the father
    • Template available here.

12. Affidavit to Use the Surname of the Father (AUSF)

    • Four (4) original affidavits, each notarized by a notary public and apostilled by the competent authority
    • (IF CHILD IS BELOW 7 YEARS OLD) Executed by the mother
    • (IF CHILD IS 7-17 YEARS OLD) Executed by the child and attested by the mother
    • (IF CHILD IS 18 YEARS OLD AND ABOVE) Executed by the child

13. Affidavit of Delayed Registration

    • One (1) original and three (3) photocopies of a notarized affidavit explaining the delayed registration
    • FOR THOSE REPORTING ONE YEAR AFTER THE CHILD'S BIRTH, KINDLY ACCOMPLISH THE AFFIDAVIT BELOW:

14. Dual Citizenship Identification Certificate

    • Four (4) photocopies of the child's dual citizenship Identification Certificate
 
 
 
  NEXT STEP  
 

After your Report of Birth is approved, you can take several actions that will facilitate various administrative processes:

1. Apply for a Philippine Passport. You have the option to get a Philippine passport.

You can use the Report of Birth for your passport application if it was issued within 12 months by the same Embassy/Consulate. If your Report of Birth is older than 12 months or from a different Embassy/Consulate, you cannot use it and will need to obtain a PSA copy.

2. Get a Certified True Copy. You can get a certified true copy of your Report of Birth issued by the Philippine Embassy.

3. Request Transmittal Details from the DFA. The Embassy will forward the approved Report of Birth to Manila to become a permanent record of the (PSA). Please request the transmittal details (Reference Number, Dispatch Number, Dispatch Date and Transmittal Date) from the Department of Foreign Affairs-Consular Records Division (DFA-CRD) through oca.crd-us@dfa.gov.ph.

4. Obtain a PSA Copy. After six months from the date of approval of the Report of Birth, you must request an authenticated copy from the PSA through psahelpline.ph or psaserbilis.com.ph. You will need the transmittal details provided by the DFA to complete the online order.

Most Philippine government agencies will only accept a PSA copy of birth certificates for public transactions.
